The butterfly yoma from Curse of the Undead Yoma. 
As far as I can tell, she has no name. I've only 
ever seen but a handful of cels from this movie 
at all, and only ones of the main good guys.

The movie isn't *incredibly* old, but it's far 
from new, so I don't know if this counts for the 
thread or not. ^_^;
