Actually, the customs form for the UK is a 
large white one, not the green one, that fits
into a plastic slip attachment.  (At least for
air parcel post... not sure about ground or
uninsured shipping methods...)  

If you ask, they'll give you one.  Most of the
newer systems will require them to scan the
form anyway when they process the shipping on
your package.
