About the two backgrounds question, many cel setups require
another cut-out background on the top to make the layering
effect.  For example, if there is a scene that showing a 
girl sitting inside a car.  It may setup with a 
large background on the bottom layer, the cel is placed 
on top, then another cut-out background of the car is placed
on top of the cel.  In other words, the cel is sandwiched
between two backgrounds, one full background and a cut-out
background.
About your second question, some images are scanned  
poorly, especially with light colored backgrounds.  Many
people like to adjust the brightness/contrast setting after
the scanning, if they turn the brightness too high, the
color of the background will wash out and all you see is
either white or light yellow color.
